The purpose of this study is to implement a nurse-coordinated model of follow-up care, called IBIS-survivorship, where the responsibilities for your care are shared between your cancer specialists (surgeon, oncologist, oncology nurse) and your general practitioner.
This study is designed to find out if IBIS-survivorship can improve your quality of life and experience with health care during your post-treatment follow up.
