If you are offered the IBIS-Surviviorship model of care, a copy of the Survivorship Care Plan that you developed with your Specialist Cancer Nurse will be sent to the GP and discussed between your GP, the practice nurse, and your Specialist Cancer Nurse. The Survivorship Care Plan includes a summary of the treatments you had, advises your GP how often to see you, and lists common side-effects of treatment to look out for, any tests or examinations you may need, and who your GP can contact if they have any questions or concerns. In addition, it will include the health and wellbeing goals you developed with your Specialist Cancer Nurse. Your GP plays an important role in your Survivorship care as they are familiar with your health overall and are skilled at monitoring it closely and they can work with you to achieve your goals and improve your general health and wellbeing.
If you are offered Usual Care model of care, then it will be up to your health care team how they provide information to the GP.
What if I’m going on holiday during the study?
Please contact the study team or your Specialist Cancer Nurse and they will schedule your questionnaires, interview or face to face appointments around your holiday.
You will be actively participating in study activities for 12 months (i.e., answering surveys). After this time, you will not need to actively complete surveys, but the research team will continue to access your health data for up to 5 years.
What do I have to do on the study?
The study team will contact you when you start the study and at 6 months and 12 months after you completed your primary treatment for early breast cancer to ask you to complete a number of questionnaires. The questionnaires will take about 20 minutes to complete, a little longer if you opt to complete them by phone.
If you indicated your interest when you consented to the study, the study team will contact you after you have completed your 12 month questionnaires to ask you to participate in a one off, individual interview to find out about your experience of participating in the trial. The interview should take no longer than about 30 minutes.
Do I have to visit the hospital while on the study?
You will still need to visit the hospital for any planned face to face appointments with your Specialist Doctors or Nurses but you can complete the study questionnaires and interview at home (telephone or online).
