Optimising Outcomes and reducing health inequities for heart failure patients in their last year of life in Queensland

This research study provided evidence to understand hospital-based service use for individuals who died of heart failure and cardiomyopathy between 2008 and 2018 in Queensland. Specifically, the study sought to examine patterns of hospital admissions, including admissions to an ICU, comorbidities and palliative care hospital admissions as well as ED presentations.

The study was a data linkage study, conducted using administrative health data relating to hospitalisations, emergency department visits and deaths.

Project outcomes: The population-level data in this study is critical for planning health services, workforce and care provision which enables improved access to palliative care. The data is important to take into account when designing and assessing models of care for distinct clinical diagnoses.
